Jenis pengukur ini tidak perlu diisi cairan untuk menghindari guncangan internal (ketidakstabilan tekanan media pengukur) dan guncangan eksternal (getaran yang dihasilkan selama pengoperasian peralatan) agar tidak memengaruhi pengamatan atau menyebabkan kerusakan pada roda gigi. Pengukur tekanan diafragma tanpa roda gigi juga dapat menahan benturan tinggi. Kerugiannya adalah presisi yang buruk. Ketika tekanan terisi, bellow akan memanjang di sepanjang arah tabung, menggerakkan roda gigi dan tautan untuk memutar penunjuk. Selain sebagai pengukur tekanan umum, alat ini juga sering digunakan sebagai pengukur tekanan diferensial. Prinsipnya adalah menghubungkan bagian bawah pada kedua sisi media tekanan secara terpisah. Melalui perbedaan tekanan antara kedua sisi, penunjuk akan berputar. Alat pengukur ini biasanya digunakan untuk mengukur perbedaan tekanan mulai dari 0 ~ 25 kgf\/cm\u00b2. Tekanan statis maksimum adalah 150 kgf\/cm\u00b2. Bahan bellow yang umum digunakan adalah baja tahan karat, kuningan, dan perunggu.
